چکیده

Four days after she began enoxaparin, a tender, firm, hemorrhagic vesicular rash, ranging from 0.1 to 0.8 cm, developed on her calves bilaterally (Figure 1). The rash bled with minimal trauma and dried into crusted scabs. These lesions were distant from the enoxaparin injection sites on her upper arms. Laboratory results included: hemoglobin, normal; leukocytes, 19×109/L; platelets, 353,000/μL; prothrombin time, 21.8; INR 1.8. Differential diagnoses considered were leukocytoclastic vasculitis, warfarin-induced necrosis, bedbug bites, enoxaparin dermatosis. A shave biopsy specimen was obtained for histopathologic examination. It showed noninflammatory subepidermal bullae consistent with bullous hemorrhagic dermatosis (Figure 2). There was no evidence of leukocytoclastic vasculitis. No thrombosis was seen as might have been evident in warfarin-induced necrosis. This is consistent with enoxaparin dermatosis. This is a very distinct, rare, and remarkable hemorrhagic rash, first recognized in 2006. It is crucial to diagnose it correctly because it may require cessation of culprit heparin or heparin derivative, especially if the rash is worsening. Notable is the fact that the rash does not occur at the site of injection but rather at sites distant from heparin use. The exact cause is unknown. Existing case reports describe a self-limited course that resolves within several weeks without interruption of treatment or other adverse events.

Bullous Hemorrhagic Dermatosis Induced by Enoxaparin

Bullous hemorrhagic dermatosis induced by enoxaparin is a rare, self-limiting, cutaneous adverse reaction causing no complications. In this report, we present a case where bullous hemorrhagic dermatosis developed at a location distant from the site of injection after using enoxaparin for 5 days for pulmonary venous thrombosis.

Bullous hemorrhagic dermatosis due to enoxaparin use in a bullous pemphigoid patient

Adverse reactions of subcutaneous low molecular weight heparin or unfractionated heparin could be complications by bleeding, heparin-induced thrombocytopenia, drug-induced liver injury, osteoporosis, and cutaneous reactions. Heparin-induced skin lesions vary from allergic reactions like erythema, urticaria, eczema to intradermal microvascular thrombosis associated with heparin-induced thrombocy...

Bullous hemorrhagic dermatosis at distant sites: a report of 2 new cases due to enoxaparin injection and a review of the literature.

Enoxaparin is a low-molecular-weight heparin used in the prevention and treatment of pulmonary thromboembolism and other thrombotic disorders. The most common adverse reactions to enoxaparin are ecchymosis, skin necrosis, urticaria, angioedema, and eczema. The first 2 cases of bullous hemorrhagic dermatosis in areas distant from heparin injection sites were described in 2006. We present the cas...

Association of linear Morphea and perforating dermatosis: A case report

Linear morphea is a distinct form of morphea that occurs as a linear band usually with a single unilateral distribution. The lower extremities are most often involved. This dermatosis appears as a linear induration not necessarily related to the derm induration. We report herein a case of 20-year-old Iranian girl who had a linear sclerotic patch on right lower extremity with clinical and ...

Effects of Enoxaparin Emulsion on Dimethylbenzanthracene-induced Breast Cancer in Female Rats

Background : Enoxaparin is an anticoagulant medication. Anticoagulation inhibits tumor cell–mediated release of angiogenic proteins and diminishes angiogenic response. Angiogenesis is an important event in various cancers such as breast cancer. Angiogenesis provide oxygen and nutrients to tumor cells and causes tumor progression. The aim of the present study was to evaluate the anti-angio...
